Procedure for a Hesperia Dog License

Municipal Code 6.04.010 requires that all dogs over four months of age must have and wear a current and valid license tag issued by the City of Hesperia.

1. A current rabies vaccine is required to license your dog. Low price rabies clinics are held by Hesperia Veterinary Services each Wednesday from 9am - 11am and 2pm - 4pm. For more information, call 760-244-3833.

2. Complete the application on the back of this form and submit form and fees to Hesperia Animal Services. Fees can be mailed in or brought to Hesperia Animal Services. Do not leave blank spaces or license cannot be processed. You must attach the required certificates to this form to receive a license. (Current rabies certificate, alteration certificate and proof of age 55 or older, if applicable.)

3. Within 1-2 weeks you will receive your permanent license and tag. If you do not receive your license, call 760-947-1700.

4. License is renewed on a yearly basis.
